  • Divorced or widowed applicants:  You or your future spouse must provide the date of the divorce or the date of death. If either of these circumstances has occurred within the last six months, you will need to provide a certified copy of the divorce decree or death certificate, both of which can be ordered from your local vital records office and mailed to you.
  • The Name Change:  After you are legally married, you will receive a marriage certificate, which is what you will need to notify government agencies of your name change.  (Woohooo!!)
